My Medicare
MyMedicare is a free and voluntary program developed my Medicare to strengthen the Doctor -patient relationship.
When you register in MyMedicare, your general practice will be notified that you have chosen them as your regular care team. Additional funding may then be available from the government to help them provide the care you need including :
- greater continuity of care with your registered practice, improving health outcomes
- longer Medicare Benefit Scheme (MBS) funded telehealth consultations with your GP ( if they offer telehealth)
- access to bulk billed care plans & health assessments

Chronic Disease - care plans
Medicare implemented a prevention program to address the burden of many chronic diseases on the health budget. Prevention and early treatment of diabetes, mental health, arthritic conditions, chronic pain, to name a few, can be funded through Medicare. If you’ve had a chronic medical condition for at least six months or a terminal disease, you and The Doctor may agree to a chronic disease management plan. This will then be booked with our Chronic Disease Nurse Melissa. Appointments available Monday , Tuesday & wednesday.
Health Assessments
This preventative health check gives us an opportunity to discuss your health needs, identify any factors that could help improve your overall health or prevent illnesses in the future.
They can also assist in identifying support services that may assist you daily living & keep you safe & well at home.
Medicare have funded these for patients 45-49 years & over 75 years. They have also funded heart health checks for all patients over 30 years.
Over 75 year old Driving medicals
Once you turn 75, the law states you must have a medical clearance to continue to drive.
It is our policy is that everyone renewing their drivers licence requires an annual health assessment. Always ensure you allow time to have this performed & processed prior to your licence expiration date.
